Please arrive 15 minutes before the assigned time. The person coming for a health check has to be healthy and can not have a valid certificate of incapacity for work.
Please fill out the health check card beforehand and take it with you when coming for your health check. If blood tests are needed then please do not eat or drink (unfavoured still water is allowed) 3 hours before the health check for more accurate results. If you have any prescription medicine then take those as your doctor has ordered. If you also need to go see an optometrist and you normally wear lenses, then please remove your lenses the previous night and wear glasses for the health check.
You can fill out the necessary paperwork prior to your appointment.
